E46 constantly going into limp mode

hey guys im hoping that you can shed some light on my predicament. My 2004 320i is constantly going into a fail safe/limp mode and it seems to trigger whenever i am at the stop lights or jus sittin in traffic.I had a diagnostic run on it and it gave me the error codes p0720 stall speed, p0741 torque converter,p0721 output speed sensor, converter clutch lockup excessive slippage. I changed my transmission control module,the Air Flow sensor, changed the transmission fluid and still no change.

Maybe you need to have a look at your torque converter ...
