I cannot update my signature. Can someone please help me change my current signature to the one below? thanks

Unable to add attachment as Image icon only by URL. My Media is empty (attachments was not transferred from previous DB) and when try using Manage Attachment @ My Settings, there is no options to add media. Besides that, there is no Preview feature before saving a signature....
